The CHD clinic is run by Specialist Nurse Practitioner Manager, Elaine James.  The lead doctor is Dr Smith.

We run a Coronary Heart Disease (CHD) for patients with or at risk of cardiovascular disease, for example those patients who have suffered a heart attack or experience angina, patients that have had a stroke, and also patients  that have a higher risk of developing Cardiovascular Disease, such as those with Diabetes with hypertension (high blood pressure).

Your GP will refer you to the CHD Clinic or you will receive a letter direct from the CHD Team. We can help you to prevent  or manage cardiovascular disease, support you with lifestyle changes, screen for complications and make changes to treatment where necessary.

We encourage patients to take an active part in managing their health. If you would like to monitor you blood pressure at home we would encourage you to do this and let us know your results. Blood pressure monitors can be purchased from your local pharmacy or online. The link below explains the importance of blood pressure and there is a video by the British Heart Foundation that shows you how to take a blood pressure reading. There is also a blood pressure diary sheet that you can download to record your readings. Bring this in when you attend your CHD appointment.

For patients that have been diagnosed with a heart condition, Redditch Hale and Hearties Coronary Support Group is also available to help – see details below: 

We are here to provide support for people who have a heart condition, support is available for both patient and their partner or carer.

Membership is free and all are welcome at our monthly meetings.
